DUTIES:
This individual functions as a Social Worker on a

multidisciplinary case management team located at the

Huntington VA Medical Center. The incumbent serves, in the

community, those Veterans with severe and persistent mental

illness who have frequent or long-term hospitalizations.

The client population's illness is chronic in nature,

refractory to resolution and requires monitoring to

maintain stability. The incumbent will individually tailor

services to meet the needs of the veterans and deliver them

on a one- on-one basis or in a group setting. The incumbent

will assure continuity of care by using a team approach and

sharing responsibilities.

The incumbent is expected to provide complex clinical

services to clients with serious psychological problems on

a regular and continuing basis, including suicide

prevention, crisis intervention, short and long term

treatment, emergency location of food and shelter, legal

referral, employment, structured placement, financial

matters, socialization and any other problems encountered

in daily living. The incumbent will achieve program goals

through frequent home visits, provide medication

monitoring, crisis/stress management, as well as develop

linkages to community resources. The incumbent will work

with Veterans that have a severe and persistent mental

illness such as Schizophrenia; Bipolar Disorder, or Major

Affective Disorder and they have been hospitalized at least

30 days during the past twelve months and live within a 30

mile radius of the Huntington VA Medical Center. Incumbent

provides veterans and their family members with community

outreach, counseling therapy treatment and direct services

and referral to VA and community resources. The incumbent

employees a variety of psychological, rehabilitation,

social work, counseling and mental health techniques

including but not limited to individual, group, vocational,

educational, and substance abuse.

The incumbent coordinates

care with other VA programs including HudVash,

Homelessness, Compensated Work Therapy, and Substance Abuse

Treatment Programs. The incumbent is required to exercise

independent judgment and decision making in the formulation

of the treatment plan with the treatment team. Additional

responsibilities include an ongoing assessment of the

Veteran's progress and treatment, as well as developing

discharge plans. The incumbent provides outreach services

to clients and engages in community networking which may

include coordination with lawyers, board and care home

operators, physicians, clergymen, and representatives of

other disciplines who are concerned with the same clients

or group of clients. The incumbent performs administrative

functions such as actively participating in clinic

meetings; maintains accurate records of his/her clinical

interactions; and collaborates with other team members and

with professionals, paraprofessionals, volunteers, and

others in VA facilities and community agencies to help

veterans recognize and resolve their problems and needs.

The incumbent may provide training experiences for graduate

social work trainees; may make community visits needed to

effectively perform his/her responsibilities, may

participate in research and pilot studies, and will

maintain an active participation in the Mental Health Service

.

FUNCTIONS OR SCOPE OF ASSIGNED DUTIES

Participates in the admission/screening process by

interviewing patients and/or other stakeholders and

completing intake data collection with purpose of

establishing treatment for Severely Mentally ILL (SMI)

psychiatric disorders in a case management setting. This

may include obtaining psychosocial history, maintaining

data sheets, completing high risk screens and safety plans,

answering consults, and coordinating discharge planning

with other VA and non-VA inpatient psychiatric facilities. Participates in or facilitates selected supportive groups

related to the treatment of specific SMI psychiatric

disorders, conducts individual counseling and other social

work privileged functions independently or under

supervision as professional license dictates. May conduct psycho educational activities following

professionally prepared resource guides. Functions as a primary manager (case manager) for a number

of Veterans on a prescribed caseload of SMI Veterans. This

includes preparation and implementation of treatment plans

developed in concert with the incumbent's multidisciplinary

team. Acts as a liaison between staff, Veteran and stakeholders

in the development and review of treatments plans. Participates in periodic review of ongoing treatment of

patients under the care of the MHICM team. Provides ongoing documentation of patient's progress using

a behavioral framework and treatment plan updates. Help Veterans (1) adjust to current living situation, (2)

explore areas of deficit in activities of daily living, (3)

provide and/or arrange skill building educational

activities (4) explore areas of psychosocial conflicts or

trauma seeking resolution or psychological stability

(maintenance) by using a 'Recovery Model" based approach. Use supportive/facilitative techniques with Veterans and

their stakeholders to help them better communicate with

each other and, when necessary, help confront problem

behaviors. Acts as a resource and liaison person to public and private

providers and agencies to assure continuity of care and

appropriate referrals for Veterans and their stakeholders

as levels of care change. Conducts or assists in carrying out planned QA studies or

research projects Other duties as assigned.

QUALIFICATIONS REQUIRED:
The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must:

Be a citizen of the United States.

Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to practice at an masters level.

Have a masterÂ's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the masterÂ's degree in social work.

FOREIGN EDUCATION REQUIREMENTS: To receive credit for education completed outside the United States, you must show proof that the education has been submitted to a private organization that specialized in interpretation of foreign educational credentials and such education has been deemed at least equivalent to that gained in conventional U.S. education programs.
