MPI Identity Coordinator Summary: This position works under the direction of the Manager of Technicians and is respon! sible for maintaining the data integrity of the facility and enterprise Master Person Indices (MPI). The individual provides resolution of urgent MPI duplicate/overlay errors (those that impact patient care), non-urgent errors and resolution of potential duplicates. The incumbent will monitor and work daily reports and notifications. This individual provides resolution of urgent MPI Contact Moves/overlay report (those that impact patient care), non-urgent errors and resolution of contact moves. The MPI Identity Coordinator will work closely with the EMPI team to identify areas of potential risk, problem resolution, and communication/education with internal and external entities. The incumbent uses established fact finding approaches, knowledge of pertinent work processes and procedures, and familiarity with HIM/IT standards to research and make a determination concerning potential duplicates. This position is responsible for independently carrying out fact finding and analy! sis to assist the Identity team members in preparing detailed ! specifications and proposals for modifying the existing system when the solutions follow clear precedence. A wide variety of internal and external professional relationships are developed to accomplish departmental and hospital objectives. Developing and maintaining good communication with these contacts is essential to accomplishing the goals. To ensure compliance with established policies and procedures in patient identifier formatting and to decrease the creation of double numbers and Contact Moves across the enterprise, the incumbent will provide feedback to Merlin Training Team for adding and updating patient identifier information. Individualized re-training will be coordinated with Clinical and Registration Mangers as needed. Essential Duties and Responsibilities: include the following. Other duties may be assigned. MERLIN RELATED RESPONSIBILITIES: • Work newly identified duplicate/overlay cases using designated reports, e-mail communications, phone calls, and inte! rdepartmental mail o Obtain medical records from clinic/unit, medical files, microfilm, State Records Center (purged medical records), and other areas as needed. ? Pull, identify charts with pending requests and re-file charts as needed ? Medical records not located are declared as “lost” o Maintain log of receipt notice and medical record requests o Perform review of medical records to determine duplicate/overlay utilizing established review form and procedures ? Review all medical records involved to compare patient demographic and medical information ? Utilizing various external databases, confirm accuracy of patient demographics ? When above sources do not accurately confirm duplicate cases,the incumbent will contact the patient if possible, ? Follows established procedure for combining cases or adding known non-duplicate patient indicators to the EPIC systems. o Merge cases in EAD Enterprise Access Directory o Notify all affected users of merge movement o Log outco! me of review and reply to original notification contact of outcome • ! Additional steps involved for overlay and active inpatient identification/corrections o Prior notification to all designated staff o Creation of new medical record number o Work with clinical staff to move active appointments and referrals. o Move designated cases in EPIC established procedures • Review and process information collected ensuring corrected information is viewable in Horizon Patient Folder and Epic systems. • Process EAD information searching for incorrect data. o Correct identified errors • Prepare reports for distribution and monitor for completion o EAD Reports o EPIC Reports o User formatting error Report • Review and resolution • Assist all staff with calls as they relate to double numbers • .Work newly identified contact moves/overlay cases using e-mail communications, phone calls, and interdepartmental mail and designated reports o Review source and target patients o Coordinates correction of contact moves throughout all inpatient and outpa! tient information systems, including all downstream systems. • Specializes in data correction for entire wrong contact and single note documentation errors • Facilities clear communication channels when charting errors occur • Collaborates daily with the management, registration, clinical staff, trainers with information of expectations of data corrections Works newly identified issues resulting from patient mix ups by researching the patients history Coordinates corrections of data entry errors and patient mix ups throughout all inpatient and outpatient information systems, including downstream systems. Review and correct patient identifying information in registration, scheduling, record tracking and clinical systems so that all systems reference the correct patient and duplicate identifying information is eliminated or that the impact of duplicate or indexing problems do not effect patient care and appropriate electronic patient documentation. Along with the electr! onic system corrections, update any associated paper documentation to b! e consistent with the electronic systems. LEGACY SYSTEMS RESPONSIBILITIES Assume primary responsibility for the correction of duplicate patient registrations (duplicate CPI). • Analyzes each duplicate number assignment to identify most appropriate number to utilize. • Requests patient record and/ or microfilm and combines all patient records, re-numbering and re-assembling the permanent record appropriately. • Assumes responsibility for timely, accurate combination of accounts in the Patient Management and TDS information systems. • Facilitates correction of double numbers throughout all inpatient and outpatient information systems to include, but not be limited to: Clinical Laboratories (Sunquest), Radiology, Pathology, Family Medicine, Decision Support, POC Medical Records, Utilization Management, Patient Access, Information Technology, Patient Accounting, Cancer Center, Cardiology, Medsite, and Medical Transcription Departments. • Concurrently monitors originat! ion points of duplicate number assignments, reporting same to appropriate registration area for educational use. • Collaborates daily with the following persons to ensure integrity of data throughout the critical information systems: Supervisor, Professional Billing; Hospital BillingManager, Enterprise Applicatons (TDS); Supervisory, Laboratory Information Services (Sunquest); and User Support Specialist (PM). Perform software and scanner maintenance setup daily by work assignment. • Initiate and activate scanner by using Microsoft Windows daily. • Assess and evaluate scanner set-up properties based on work assignment. • Must understand the difference between feeder and flatbed adjustments. • Reset scanner based on specific work assignment (i.e. colored documents). • Performs daily maintenance (i.e. paper jams) and weekly cleaning of scanners. Scans and indexes medical records for optical imaging and routes finished documents to appropriate areas. • Selects ap! propriate imaging cabinet based on assignment. • Scans documents by p! lacing in optical scanning equipment. • Downloads data in existing digital format when necessary. • Indexes documents with patient identifying information as determined. • Loads scanner with documents based on size, color, type, etc. • Assure that documents meet quality standards and accept or reject. • Re-scans documents when necessary to ensure clarity and accuracy. • Utilizes windows technology to select drop down boxes. • Communication / Confidentiality • Communicates clearly and effectively both orally and in writing. • Maintains patient confidentiality by not revealing and/or discussing patient information with anyone who does not have a legitimate interest in the information. • Observes attorneys, patients, et al, reviewing records when necessary. • Monitors faxing and printing of patient records to ensure legitimacy and accuracy when necessary. • Uses good judgment and discretion when dealing with physician offices concerning access to and av! ailability of records. • Works as a team member with Management, co-workers, and other departments. Position Requirements

Education and Experience:

  • Graduate from an accredited program in Health Information Technology (HIT) preferred
  • Eligible to take the Registered Health Information Technicians Examination (RHIT)
